A roadmap for state Medicaid programs

Three CMS CRUSH mandates are now landing on state Medicaid programs. Accelerated provider revalidations. Advanced data and technology. Intensified audits of state fraud control units.

Each one requires states to accelerate the detection and prevention of provider-level fraud, waste, and abuse. And each one has a federally mandated timeline to achieve success. All three will be hot topics at NAMPI 2026 in Portland this August.

Understanding the CRUSH mandates is the easy part. Meeting them requires a commitment to one word: CONTINUOUS. CRUSH initiatives, like Medicaid provider revalidation, don’t end when a plan is in place or an initial validation process is completed—it is a continuous process.

Here is a quick summary

1. Accelerated Provider Revalidations

The April 2026 CMS directive requires all state Medicaid programs to swiftly and continuously revalidate providers deemed at high risk for waste, fraud, abuse, and corruption.

The mandate. States were given a 30-day window to submit a comprehensive, statewide provider revalidation strategy to CMS.

The goal. Force states to own the problem by verifying that only legitimate providers are actively enrolled and billing the system.

What immediate and continuous compliance looks like

A one-time sweep of the high-risk cohort satisfies the deadline. It does not satisfy the goal. Providers change ownership, licensure status, addresses, and affiliations continuously. A provider who is low risk when the strategy is filed can be high-risk months later, and a provider who exhibits high-risk behavior in one state may not yet have exhibited that behavior in another. Point-in-time processes cannot satisfy the continuous spirit of the mandate.

Compliance is not proving your providers were validated months ago—it is proving they still are today.
